Referrals
To claim your medicare rebate you will need a valid referral from your GP (generally lasting 12 months) or specialist (3 months). Please bring the referral with you to your appointment or email a copy through to info@northernheartcentre.com.au.
What to Bring
In addition to your referral, please bring your medicare card and a list of your current medications, as well as any previous pathology or imaging reports you feel are relevant.
For correspondence and referrals, the practice prefers to use email in preference to fax. Please contact us by phone for anything urgent.
